    CQC – Care Quality Commission ratings roller banner. Our banners are designed to meet the standards set out in the CQC guidelines document.

    Choose your Care Quality Commission rating banner size, your rating, and the name of your care home or establishment, and also you can add a logo if you have one.

    Please fill in the above form adding your values in the relevant areas of the form.
    1. Choose the correct size:-
    600mm wide x 2000mm high visible area @ £65.00 each
    800mm wide x 2000mm high visible area @ £78.00 each
    2. Add your company logo. This is optional but please try and get the best image you can so we can print it. Normally you can find a copy of your logo from any brochures or literature that have been printed.
    3. Add the name of your care home or establishment as you want it to appear on the certificate.
    4. Add your overall CQC grade. Please don’t add one element of your grading just the overall grading achieved.
    5. Add your grading date.
