What is the block made of?
The sign holder is manufactured from high quality diamond polished Supervue acrylic. The sign holder boasts a 1.5cm clear acrylic base.
Please fill in the above form adding your values in the relevant areas of the form.
1. Add you company logo. This is optional but please try and get the best image you can so we can print it. Normally you can find a copy of your logo from any brochures or literature that have been printed.
2. Add the name of your care home or establishment as you want it to appear on the certificate.
3. Add your overall CQC grade. Please don’t add one element of your grading just the overall grading achieved.
4. Add you grading date as Day | Month | Year e.g. 17 July 2021.
